Create and send assessments

7 min read Updated October 16, 2026

Assessments are questionnaires your clients fill in, such as the GAD-7 for anxiety or the BDI-II for depression. You can use one of mePro’s ready-made assessments, copy one and change it, or build your own. Then you send it to a client, once or on repeat.

Note: All clients, names, emails, appointments and other data shown in the screenshots are sample data we created for this guide. They are not real people or real records.

When to use this

  • You want a client to complete a standard measure, such as the GAD-7 or BDI-II.
  • You want to track a client over time by sending the same assessment regularly.
  • You want a questionnaire with your own questions.

The Assessments page

In the left sidebar, click Assessments. The page has two parts:

  • My Assessments: the assessments you’ve made or copied. Each card shows its categories and how many clients it’s been sent to, for example 1 Client.
  • mePro Templates: ready-made assessments, such as DBT Diary Card, Mental Status Exam (MSE) and Beck Depression Inventory–II (BDI-II). Each card shows how many times it’s been used.
The Assessments page.

Find an assessment

  • Search: click Search assessments and type the name of the assessment.
  • Filter: click the filter icon next to the search box, then tick one or more categories, such as Adults, Couples, Scored Measures or Self-report Questionnaires. Only assessments in those categories are shown.
Click Search assessments and type a name.
Or click the filter icon and tick categories.

Create your own assessment

Step 1: Fill in the assessment details

1. Click + New Assessment at the top right.

The Create Custom Assessment window opens, with empty Assessment details at the top and Build your form below.

Click + New Assessment.
A new, empty assessment.

2. Enter a Title and a Description.

Title: the name of the assessment, for example Client Wellbeing. The box suggests names such as PHQ-9 or Custom Intake Form.

Description: a short description for your own reference. It also shows in the template list.

Title and Description filled in.

3. Optional: add a Note for clients.

This is shown to clients with the assessment, for example asking them to answer as openly as they feel comfortable.

Title, Description and Note filled in.

4. Click Category and tick one or more categories.

General is selected to start with. You can also tick categories such as Infants, Children and Adolescents, Adults, Seniors, Family, Couples or Scored Measures. Scroll for more. The categories show on the assessment’s card and help you filter later.

Choose one or more categories.

Step 2: Add questions

Under Build your form, the Toolbox on the left lists the question types. The middle is your form, and Properties on the right shows the settings of the question you’ve selected.

1. Drag a question type from the Toolbox into the middle of the window.

For example, drag Text Input. The question is added with a starting title, such as New Text Input, and its settings open under Properties.

Drag a question type into the form.

2. Click the question and set it up under Properties on the right.

Title: the question itself, for example What name would you like your therapist to use?

Description (optional): extra help shown in smaller text under the question.

Required: every type of question has this switch. Turn it on if the client must answer; a red star (*) then appears next to the question. Turn it off to make the question optional.

The Title is the question clients read.
The Required switch, under Description.

3. Set the question’s own options. See Step 3 below.

4. Add the rest of your questions.

Drag more question types from the Toolbox into the form. To start a new part of the assessment, click Add Section. A new section appears with its own title, for example New Section, and you can drag questions into it.

5. Change the order if you need to.

Drag a question up or down by the handle (six dots) on its left. Sections can be moved the same way, or with the up and down arrows at their top right.

To copy a question or section, click the copy icon at its top right. To delete it, click the trash icon.

Step 3: Set up each question type

Every question has a Title, an optional Description and the Required switch, which you can turn on or off for any type. Each type also has its own settings:

Text Input

A short typed answer, such as a name. Under Text Options:

  • Input Type: the kind of answer, for example Text.
  • Placeholder: gray example text shown in the empty answer box, for example your name.
  • Max Length: the most characters the client can type.
The Placeholder shows in the answer box. Required is on, so a red star shows next to the question.

Yes / No

A single yes-or-no question, such as Have you experienced difficulty sleeping recently? Under Boolean Labels, change the words on the two answers with True Label (for example Yes) and False Label (for example No).

True Label and False Label.

Rating

The client picks one number on a scale, such as How would you rate your overall emotional wellbeing this week? Under Rating Options, set the lowest number (Min) and highest number (Max), for example 1 to 5. Use Min Description and Max Description to say what the ends mean, for example “Very low” and “Very high”.

Min and Max set the scale.

Scale Rows

Several items rated on the same scale, such as How frequently have you experienced the following symptoms during the past two weeks? Set Min and Max for the scale, for example 1 to 12. Each row is one item, such as Anxiety: type it in Label. Click Add row for another item, or the trash icon to remove one.

Scale Rows: one row per item.

Yes / No Rows

Several yes-or-no items under one question, such as Have any of the following situations affected your daily life recently? Set the answer words with True Label and False Label, then add one row per item, such as Difficulty managing stress. Click Add row for more.

Yes / No Rows: one row per item.

Long Text

A larger box for a longer written answer, such as What are your main goals for therapy?

Radio Group, Checkboxes, Dropdown and Ranking

Questions with a list of answers. Radio Group and Dropdown let the client pick one answer, Checkboxes lets them pick several, and Ranking lets them put the answers in order. Set the answers under Choices: type each answer in Label, click Add Choice for more, or the trash icon to remove one. You can also turn on Show "Other" option or Show "None" option.

Choices.

Step 4: Preview and create

Preview shows the form as clients will see it.

2. When you’re happy, click Create Assessment at the bottom of the window.

The Complete button you see in the preview is the one clients will use; you don’t need to click it. Your assessment is added under My Assessments. Click Cancel to close without saving.

At the end of the preview, click Create Assessment.

Use a mePro template

To use a mePro template, make your own copy of it. The copy is added to My Assessments, where you can send it to clients.

1. Under mePro Templates, click the three-dot icon (⋯) on the assessment.

A menu opens with View and Use Template. View lets you look at the assessment without changing anything.

Click ⋯ on a template.

2. Click Use Template.

A copy of the assessment is added under My Assessments, with (Copy) after its name, for example DBT Diary Card (Copy). You can now send it to clients.

Click Use Template.
Your copy under My Assessments.

Send an assessment to a client

1. Under My Assessments, click the person icon on the assessment’s card.

The Assign to a client window opens, with the assessment already filled in.

Click the person icon.

2. Under Repeat assessment, choose how often to send it.

Leave it Off to send it just once.

To keep sending it, choose Every week, Every 2 weeks, Every month, Every 2 months or Custom. As the window says, a new one is sent after the client completes the current one.

Repeat assessment options.

3. Under Select Clients, click Search client… and choose the client.

Type a name to search. Each client shows their email and client ID. The Assessment box below already shows the assessment you’re sending.

Choose the client.

4. Choose a Due date, then click Send To Client.

Click Due date and pick the day the client should finish by. Send To Client turns blue once you’ve chosen a client. After you send it, the card shows how many clients have it, for example 1 Client.

Click Send To Client.

Tip: You can also send assessments when you add a new client, or from the client’s Chart tab. You see their answers under Assessment & Forms on the Chart tab. See the Client Profile: Insights & Clinical Chart guide.
